Data and purposes
The form collects your name, email, optional telephone number and message to assess your enquiry, reply and discuss appointments. It does not confirm an appointment or provide a remote diagnosis. Do not submit identity documents, test results, medical reports or detailed health information.
IP addresses, request times and browser/access information may be processed for server operation, troubleshooting and abuse prevention. The application uses session verification and IP-based request limits.
Collection and legal grounds
Data are collected electronically through the form and browser/server operations. General enquiries and security rely on legitimate interests under Article 5(2)(f) of Turkish Law No. 6698, subject to your fundamental rights; appointment requests directly related to establishing a service relationship rely, where necessary, on Article 5(2)(c). Legal obligations and the establishment, exercise or defence of rights are addressed under Articles 5(2)(ç) and 5(2)(e). Optional consent-based measurement relies on Article 5(1). Acknowledging this notice is not consent to advertising or health-data processing.
Recipients and external services
Authorised personnel may process enquiries; hosting and technical support providers may access data as needed, and competent authorities may receive data upon lawful request. Email enquiries involve Microsoft/Hotmail infrastructure. Maps, videos and permitted measurement can involve external providers and systems abroad. Transfers abroad require applicable conditions and safeguards under Article 9; this notice or a cookie choice alone does not establish a transfer safeguard.
Retention
Retention must be limited to handling the enquiry and applicable legal or rights-protection requirements. Relevant criteria include the nature of the request, legal obligations and limitation periods. When processing grounds cease, data are deleted, destroyed or anonymised. You may contact the controller regarding retention and erasure.
Your rights and requests
Under Article 11 you may learn whether data are processed, request information about processing, purposes and recipients in Türkiye or abroad, request correction and, where conditions apply, erasure or destruction and notification of recipients. You may object to adverse results based exclusively on automated analysis and seek compensation for unlawful processing.
Send written requests to the address above. Email requests may be sent to v.tektas@hotmail.com from an email address previously provided and recorded in our system. Other requests may require identity verification under the applicable procedure. Include the necessary identity and contact information and your request, without unnecessary medical documents. Requests are answered as soon as possible and within thirty days; additional costs may be charged only under the applicable Board tariff.
